want 2 radio buttons on checkout

hi, we charge $1 extra for pobox delivery. i want 2 radio buttons on checkout where users can just either click pobox or home. if they click pobox, $1 will be added to the total. Now not all products have this condition. any thoughts about my problem. any contributions in mind.

one thing to add:

i need extra 1$ charge if the weight is over 100lbs. any idea. please help...

i still need help in this. i need this that if the weight of my products is over 200lbs then give them an option for either residential delivery or a commercial delivery. if they choose residential then charge them extra 50 dollars because some residential areas dont allowbig fedex trucks to come to theei areas etc.... this is standard charge from fedex and i cant do anything. ppl start suggesting that dont use that 50$ charge etc....please understand i need advise how to do it, not a suggestion if i should use it or not. i need 2 radio boxes if weight is over 200lbs then give them an option. please help if have seen a contribution or know some codes. i m new to php.
