Adding a check box option...only that

Hey guys

 

I would like to add only a simple check box for customers to have the option to receive the package "anonymous" instead of having the package with a box marked with the store name

 

now the thing is... i am lost in those codes hehe

 

somebody ever added an option in the shipping section ?

 

if yes... what is the process to do so ?
