Adding options to an order

Hi, I am trying to add options to the order entity but can't figure out how to do this.

 

I actually want to add a checkbox and a text box to the shopping_cart.php page so that if the checkbox is checked, there will be a flag set against the order when the checkout button is pressed and, if there is any text in the text box it will also be entered against an order record. I want to do this before the shipping as the shipping cost will change based on these values. I can't figure out how to do this, can anyone help me?
