Input Fields

Good Day everyone..I'm trying to create a Quote form by modifing the contact us page. I pretty well have everything in order except that I'm trying to create a new input field for Qty's and I need help. The first question would be where in html_output.php(or does it go somewhere else) do I need to put the new code and out of curiosity where do you find the height and width settings for tep_draw_input_field and tep_draw_textarea_field. Any help would be great because I think this might eventually make a nice contrib.
