Text Box Fields as Option

I want to create an option that basically is a "Ship To Gift Recipient" Option.

 

Gift Recipients Name:

Address

City:

State:

Zip Code:

 

Message on gift card:

 

 

 

 

Then list the items and their gift recipient information on the cart and checkout pages.

 

Final Checkout Page:

Couple of ways to handle this: If all of the items they have purchased have the gift option filled out, then don't have shipping information appear. If any of the items selected don't have the gift option selected, then allow the shipping information to appear (and maybe say something like "For items that are being shipped to gift recipients".

 

 

Any ideas?
