Adding a formmail is it possible

I would like to change my default review box to say something like "Find if for Me" then link it going to a formmail where a customer who cannot find what I have in my inventory give the details about what they are looking for. Example they have five pieces of a dinner plate and need another the form would have their personal info and a description or info box to fill in and also a browse button or some means where they could attach a photo of their pattern. Is this possible and could you give some info on how to. Your reply will be greatly appreciated..
