1062 - Duplicate entry When Testing

Hi,

 

I just installed the OSCaffiliate module a few days ago and just started testing it. Whenever I enter our site using the Affliate link I get the following error.

I have tried deleting the affiliate. I have tried clearing the tables. Now I am stuck. We were so excited about the module when we found it and can't wait to use it. Any thoughts on this subject would be much appreciated.
