Duplicate Orders - Please Help

Hi There,

Just wondering if anyone can help me.

If i add something to my cart and proceed to checkout and complete the transaction. it ends up placing 2 duplicates in the database

So instead of one order number, i now have 2 both order numbers with indentical products.

 

I have just installed a new copy of OsCommerce and was wondering if this is a bug or is it my setup.
