anyone succesfull with ot_commission 1.0 installation?

 

when I install it I have this error

it make references to ot_commission-big.php but there was not such a file in the zip...

 

I mainly seek for a contribution where I can add a fixed price for Cash on Delivery fees

A cod.php file comes with the site

 

catalog/includes/modules/shipping/cod.php

but how can you configurate the fixed price then? I have not found it
