Runtime Error/ Create Account

When I click on "create acount" I get an error message asking me to debug. Does anyone know what is wrong with the create_account.php page?

 

Also, does anyone know how to make it so whn someone wants to create their account all they need is to enter an e-mail address and password with the other stuff being optional? I put the gender, date of birth, company, suburb and state (under customer details in the admin) as false, but could not figure out how to make the other stuff false as well- or at least optional.
