Mixed up customer information!

My client has had OsCommerce (2.2 ms1) running for about 2 months now and a problem they are experiencing is the wrong customer information will show up in another customer's order. For example when we look at the order that was placed in the admin area, and we see Customer, Shipping and Billing - in the customer area another customer's name appears and the Shipping and Billing information is correct.

 

The wrong customer will get the notification that the order has been placed.

 

In one incident, the wrong customer had tried to place an order and was not able to do so. For whatever reason they could not check-out. The next order that was placed substituted the this customer's information as the Customer and the Billing contact.

 

If this has been covered before, I apologize for the repeat post, but I was not able to find a thread that covered this.

 

Any suggestions would be appreciated.
