Donation in checkout?

Hello, I have a worthy question and yes I poked around before I decided to just ask, lol.

 

Ok this is what I need for my checkout. I need to be able to put in small like checkbox or something that will allow ...well here this is the text I want there maybe this will explain it.

 

:::setting up scene::: Ok, you're at the checkout ok? lol,

and perhaps you're on the payment information page in the checkout looking right above above the comment box. I would like to add this text and a drop down list (or something).

 

"Thank you for shopping with us! We here at ***** also have a wonderful donation system. If you so choose you may select one of the following organizations and ***** will donate 5% of your order to them."

 

Now I considered just adding text above the box saying please type in who you'd like to donate to or something to that effect but lets face it josie customer wants to do as little as possible and that is why I'm thinking drop down menu or radio buttons.
