Template Issues - Cannot get header block for checkout pages

I think there's some issues in the DB. I cannot get my header block to show up on the checkout pages. I see errors in my logs related to duplicate entries and other errors. I also can't seem to apply header blocks using the Wizard.

Unfortunately, there is not enough information.

What did you do with the header block before it's gone away?

Provide some screenshots from the Checkout page in the theme designer.

The error you mentioned occurred while importing the block, and I'm not sure if it relates to the issue.

Could you send me the full log files in pm?

In the checkout pages, the header block is just empty. I try to add widgets and it will not accept anything. 

I think it's a bigger issue. I have never been able to do things like launching the Smart Watch theme, it would always come out as a partial install with nothing really in the theme.

I tried to start a new theme and it seems that there are issues there too.
