Address Book - not accepting additional addresses

I have discovered that, on the Account page, if a user accesses the Address Book and tries to add an address, the Continue button makes no response. No address gets added. Similarly, in Admin:Customers, if I try to add an address in edit mode, I get this response:

Any thoughts??
