admin template & infobox

hi,

i have CE 1.0.5.0 version and found something is not right with the order, invoice and packingslip

 the admin infobox appear again refer to the image above,

 

and the invoice, even include with template_top.php and template_bottom.php, see not showing infobox

the packingslip same problem as invoice, any suggestion that may cause this problem and solution to fix it?

The invoice and packing slips look fine? They are plain pages to allow you to print them.
