problem adding a page

ive created a page called faq.php basicly i copied conditions.php and changed the text to read faq instead of it reading condions.

 

ive added a line of text to application_top and english.php and the information box

 

the link displays in the information box but when i click on the link all i get is a blank screen

its written as facts answers and questions, im sorry if this is abit vaige

 

but im sure im missed something out eg not added a piece of code somewhere.
