insert checkbox in the contact_us.php

i have ha question:

 

i want to edit the contact_us.php. So the visitors shoult be able to contact me and order a prospect with a checkbox.

 

i would create in the contact us.php - under the mailadress 4 or 5 checkboxes to order a catalog.

 

i don?t know the whay which i can do that. can anyone help me? my english is not the best...
