How to add New Field to Checkout page.

hi,

 

I would like to know how can i add a new Input Field(s) to the Shipping/Checkout page...I need to take extra information called Delivery date etc., from the customer. this is cumpolsory for me as it is a gift delivery site.

 

I am new to OSC. can any one tell me how can i do this.
