How much and how long to do the following?

Hi, got oscommerce system currently installed on server, only it is setup as standard looking oscommerce.

 

I want to sort it out so it looks how i want it, something along the lines of this :

I can design all the parts needed using photoshop and supply them, all i need is to know how much it will cost and how long?

You can change your style sheets and other features within osc. That I can't help you with. It sounds as though you are looking for someone to do it for you.
