Shipping Methods not showing and cart problem

I'm using OSC v.2.2 MS-2. Here's my problems

1.The shipping was broken. I mean the radio buttons are not showing up, and it does not see that a shipping method is selected in checkout process. I've checked out shipping module in the admin page, but there's none module that have been installed. And when i try to install module, i got blank page, i mean plain white.

2. If I go back to the home page and try to do another checkout with a different item, it wont work. However, I cleared my cookies and it worked. So, I believe it is a cookie problem.

 

Can someone help me to fix those problem? FYI, my server using PHP 5.3.3 and MySQL 5.1.69.
