Shipping is not being applied

I logged in with a bogus "customer", and made a purchase. But when I go to check out, shipping is not applied. I'm using table rate shipping, and using the stock values.

Do you have zones set up ? If so, does the 'bogus' customer you created fall into one of the zones ? Did you choose weight or price table shipping ? What does your table look like ?
