Need Help Writing MySQL Query to Export Orders Report

I received an OS Commerce backup file and am trying to export a report to Excel/CSV containing the following information:

 

Order number

Order Date

Customer Information (Name, physical (billing and shipping addresses) and e-mail address)

Line Item for every item purchased

Order Status (shipped, canceled, returned)

 

I do not care if there is extra information generated in the report. I'm hoping someone can help me with generating a MySQL query that can be run through MyphpAdmin.
