high order fee module

Is their a module for this, or can do this in coding for shopping cart?

 

My customers require to see physical fabric swatches or samples, as we offer over 150 different colors. So we offer to send by postal service to them so they can have the first 3 swatches for free, as we can absorb the costs ourselves. But they can order more than 3 swatches, we need to recoup this.

This is common among many of our competitors.
