OSCommerce mysql backend

I am trying to make some changes to the OSCommerce checkout_payment.php script. I'd like to add a text box at the top of the page that would submit a user generated number to my table in the OSC database.

 

I know how to create the table in mysql if I need to. What I'd like to know is how to add that text box to the checkout_payment script, and how to submit the value in that box to a table in the database.

 

If this can't be answered, some ideas on how to create the box, and how to connect and update values in a store database would help get me started.
