Contact Form Modification

Greetings,

 

In addition to the contact form my client wants a 2nd form on the site with various form fields, such as check boxes, etc. As I am far from being an advanced php programmer I thought I might be able to make a copy of the contact_us.php file and modify it with new form fields for my clients needs.

 

My question relates to how do I go about setting up the form handler for such a new form such that an email will be sent to my client with the submitted information?
