collection in person add on

Hi

 

I have installed collection in person add on my website and have got couple of problems

 

First one is this

 

When I select collection in person and clcik on continue, I get this error

The second problem is under shipping methods its got collection in person listed and then store pickup just under the collection in person heading but next to the store pickup I have got a 0 and was wondering how do i remove the 0

 

I have got a screenshot but it keeps failing for some reason when i try to upload it

 

Can someone please help me
