Customers losing orders after registering

Just got a email from a customer saying that she lost her order after registering..."Every time I "filled" my cart - and then hit check out - it would bring me to the screen to register. Once I did that it emptied out my cart.." I was able to do a test order and it went thru...I can't figure out why this is happening every so often...Any help would be appreciated. I'm running os 2.3.1
