DHL - Choose between SMS or E-mail notification to customer when package has arrived to point of service

Hi!

I have recently started a web shop, and I am using DHL to deliver the packages to the customers. However, DHL wants me to add some choices to my checkout page.

 

When the package is delivered to the point of service (where the customer can collect the package) the customer has to be notified by e-mail or sms (including a code) that the package has arrived.

 

What I wonder is if there is an addon/contributions that to the checkout pages adds choice of SMS or e-mail notification together with a text field where the cell phone number or the e-mail address is entered?

Or is this easy to create by myself?

 

I have osCommerce v2.3.
