How do I add a form?

Hi, I have some products on my site which are available tax free to certain groups such as disabled people.

 

I have looked at some of the contributions on tax exempt status and none seem to do exactly what I want.

 

However I have come across a site which is an oscommerce site and they do have a solution. ( http://www.mobilitysmart.cc/index.php?p=vat_check )

 

They have created 4 tax sections which are shown once the customer selects Checkout:

 

They are:

 

I do not qualify for VAT exemption

I qualify for VAT exemption

I represent a qualifying Charity and qualify for VAT exemption

I am buying for someone who qualifies for VAT exemption

 

Each selection has a radio button next to it and once the correct radio button is selected they are either taken to a form which they must type both their disability and their name for VAT declaration or if they select "I do not qualify for VAT exemption" they are taken straight to checkout.

 

If they chosse one of the other options they are taken to a form to fill in, once this is done they are then taken to checkout and the tax is removed from their cart on the valid items. Tax is still applied to items not covered including shipping costs.

 

Does anyone know how I can:

 

Create the forms for the customer to sign (type their name) before going to checkout screen?

Remove the tax for the appropriate items?

Still have the tax show for items such as shipping costs?

Where would I find the relevant add-on for the creation of the forms? I have looked myself and nothing is obvious

 

I have been searching the forum now for 3 days and although I have seen this question asked before no one has yet answered it for us.

 

I hope this all makes sense and that someone can point me in the right direction, (not off a cliff)
