some kinf of form in the procucs_info.php to add some info

Hello im pretty new with php and oscommerce im wondering iff its possible

to have a kind a form in the products_info.php page?

 

i would like to add 2-3 boxes to input your info??

 

1.input field imei number

2.barred to and drop box menu here with different operators

3. drop down box with diffrent phone models

 

and a when they click buy now i get all the info after chackout

 

 

would be nice to have that so the customers dont have to write this in the comments field on checkout..

hi cris i tried it out,but seems that the files in this package is a normal os commerce desig so it screewed my store design,if somebody have the code and which files to edit it would be much better for me...thanks
