CoolMenu appearing on far left

I've just donr a clean install of CoolMenu and all appears to be working except for the placement of the menu itself. It is not in the left column position but instead is floating off by itself near the left top corner of the page. I see others have posted this same issue and haven't gotten replies. Does anyone have any ideas?

I also notice that there is a catalog/includes/classes/boxes.php file in the installation package but the instructions do not say to do anything with the file.
