How do I add state to address

when I sign up for an account there is no entry area for the state. Under Your address: it asks for:

street address:

Post code:

City:

Country:

 

I need it to be:

street address 1:

street address 2:

City:

State:

zip Code:

 

How do I get it to do this? Do I have something checked wrong?
