Multiple contact us forms

Is it possible to have more than one contact us form on an oscommerce site? Client needs one standard contact us form and another specifically for visitors to contact them about advertising possibilities. If so how can I do it? Also is it possible to add other types of form fields to the contact page such as some way to give the visitor a choice from a list, check boxes, radial buttons, dropdowns and such?

Yes, any of that can be done, some is already available in add-ons some you will need to write new code for, have a look through the add-on section.
