Orders with other people's info

I've just taken over support for a website using OSCommerce and a weird issue has popped up (it has happened twice in the last two months). A person will place an order and then when the next order is placed, the previous person's info is swapped into the order. It doesn't matter about the time frame (one was a few moments later; one was over 24 hours later). Now, like I said it doesn't happen all the time, but I was wondering if anyone has experienced this before and if there is a fix for it.

often due to incorrect linking (common issue with templates as the writers know sod all, gererally)
