Uploading image on catalog side

I want certain customer groups to be allowed to add photos to their profiles on our site.

I need this to go through their account edit page, but have been unsuccessful.

 

I borrowed some of the code from the admin manufacturers edit page, however it is not passing the filename.

 

Any ideas on how I can get this to work.

I really do not want to have to add extra pages just to upload a photo, it is important for this to function in the customer account edit page.
