How to do it ?

I need Something like in web www.napiszdo.pl. There is button ( wypisuje i wysylam ) in engilsh (im write and sending). When you click inside is a address ( adresat (do kogo) and comments in english "i whis you"( treść zyczen). I need something like this in my shop in oscommerce 2.2. because im seling a gift cards and i need to every cards address and comments i wish you. People send more diffrent card to diffrent address and diffrent comments. Please help me how to do it. Sorry for my english and thank you for help. Regards Fido30 ( Pawel)
