adding technician charges when checkout from custom pages

Dears

 

I have a osc based website and i have to create a custom page. Have a look at the below fields.

 

Items Number [drop down]

color [red/blue]

Qty [Text box]

Total [qty * 100$]

 

[Add to cart]

Note : This will include the Technician charge of 150$

 

 

As i showed here, checkout should add the technician charges also. Please tell me how can i do this.
