Charge Additional % Fee for International Orders

I am wondering how to modify or use the existing shipping modules (or if there is a contribution that does what I want) to do the following:

 

I have this working currently using table rate shipping:

1.) Shipping charge is 12% of the order price.

2.) For orders under $200, a $9 low order fee is added.

 

But I need to ADD this:

3.) For orders shipping outside the U.S. or to Alaska and Hawaii, an ADDITIONAL 13% of the order price will be added (or I guess you could say shipping charge of 25% instead of 12% - but the wording of "additional 13%" is how it was put to me by the store owner).

 

I know this is ridiculous and hokey as far as charges and calculations go, but that is what the store owner wants, and I don't know how to do this. I tried to use the rate zone shipping, but then you are given a choice of shipping rates at checkout. The store owner doesn't want to give shipping rate options, just have it set up so that the extra 13% is automatically added on if you are in one of those aforementioned areas.

 

Any suggestions? I'm going crazy here! :blink:
