creating extra fields in csv

right guys got easy populate installed with extra fields contribution ready to go.

 

downloaded the extra fields csv file to create my extra fields but I'm totally lost

 

I want to add quite a few.

 

for a pint (or 2) can anybody set up a csv for me if I gave them the the fields?
