Create application form

What's the best way to create a rather long form? This would be for a finance application, which is actually already created in Cold Fusion and was working fine before we migrated to osCommerce and a non-CF web server.

You can use one of the existing forms (account_create.php) as a template to create the application form. Just remember to include the new page in the associated files (filenames.php, database.php)
