No product image in lightbox

I am having an issue where the thumbnail are visible when I click on the product image the light box comes up but no product image. This install was working but stopped recently. I am trying to determine if it is my server or the contribution but I an not sure where to start.

 

Part b is there a way to display the product images with out the lightbox like the default.

1) it's not the image:

oscthumb.php?src=/images/IMG_2474.jpg&w=400&h=600&f=jpg&q=95&hash=861b3bd1a8c1e5e312f2440a71e670c5

and the path show the error!

 

2) as your module has been incorrectly installed, all of your images have the same "name" for lightbox:

 rel="lightbox[1]

as it should be

image 1:

 rel="lightbox[1]

image 2:

 rel="lightbox[2]

image 3:

 rel="lightbox[4]

Not sure what went wrong so reinstalled from scratch commerce and the oscthumb now nothing show on the product_info page e.g. http://forsheboutique.com/product_info.php?products_id=30

Why does the oscthumb.sql not alter the able to add feilds for the addition images
