Add shipping types as table

Hi,

I need to add many shipping types at checkout_shipping.php as table: number, shipping types for different product types and price, just for custimer can choose it and write me the selected type in comments. As i see it may be 1) whole table before Comment field or 2) link to some page with this table. What is more simple - first or second and how to do it?

I can't find answer at forum or contributor area, but it exist may be.
