Adding a Text Field

I have a client that sells products that can have personalization on each product, but a field for them to input a name to be embroidered or labeled on the product doesn't exist. Only quantity and whatever you set up via the attributes (which does not have an option for free form text entry). Is there a way to accomplish this? I tried adding the html code for a field into the product description and that works, but then there's no actual way for that field to transfer over with the other data. It's just a field that does nothing.

 

I'll be perusing the add-on's but because the shop is a template, most of the add-ons we found don't have instructions that don't match up with the file structure of the original and it ends up being a nightmare. So I thought that there may be an easier way. Ideas?
