How Do I Install The Tell A Friend Infobox?

I have installed the info box, and set the sort order. It did show up for a little while but has now dissapeared.

No matter what i do i can not get the Tell A Friend Feature to stay on my main page.

Can someone tell me how to edit the source codes to put this in manually?
