Personalised Text Option

I will be selling personalised products and I will be needing the store to have the abiltity to allow customers to send through up to 5 lines of text which will go direct to the cart to match with the item.

 

To match with a product such as this:

http://integritylabels.com.au/store/produc...p?products_id=1

 

I hope someone understands what I mean.
