Need Contact Us with attached file

Hello all,

 

I need a contact us form but to be able for clientes to attach an file.

 

I have looked all over and I cannot get it to work :(

 

If anybody has sometihing similar It would be great.

OR you could include a simple upload script to the page that will save the file on your server.

There are loads of these for free around the 'net that should be not too hard to integrate
