Help! Restoring my backup isn't working!

Help! I was trying to use this contribution: http://www.oscommerce.com/community/contributions,1537 to have the ability to put one product into more than one category, and now my catalog is all messed up. (I am totally new to PHP, so I'm sure I did something wrong... I had a hard time understanding the instructions...)

 

I tried to restore the backup, with no luck. I tried doing it through admin panel/Tools, and when I click "restore" it just seems to do nothing. So then I tried doing it through mysql panel and again, it seemed to do nothing. What am I doing wrong? I used the Tools section in my admin panel to create the backup...

 

Anyone out there help? I still get this warning when I click on "catalog" or "categories/products" from my admin panel...

Did you copy the file prod_cat_header_code.php to includes/modules?

 

Might be worth checking you placed any other files required in the right place.
