Help - Migrating from PC to web hosting

I need some help quickly and would be grateful if someone can explain step by step.

 

Last year I spent a lot of time customising oscomemrce cart on my local pc,

 

My local PC has: Windows Xp. I installed Mysql, created database using phpmyadmin and then added lot of community add ons to my standard cart to the point where everything was working great. Then I got busy and left it for a while.

 

Now I want to set up a live shop with a Web hosting company – The problem is I do not remember the databases I set up and cant even access them using phpmyadmin. (At the time I ran some sql scripts as well in order to customise my databases so that I could add new features to the cart)

 

Does that mean I will have to re-design and customise shopping cart from scratch? Is there anyway I can use my already customised shopping cart and create some new database when hosting on live server?

 

I don’t remember what username password, database name etc I used?

 

Please guide me?
