Seeing Double in Admin

Hello,

 

I just setup a second osCommerce on a different server..

My problem is in the admin section in seeing double everything:

Example:

 

Title Value Action

First Name 2

Date of Birth 10

E-Mail Address 6

Last Name 2

Street Address 5

Company 2

Company 2

Post Code 4

City 3

State 2

Telephone Number 3

Password 5

Credit Card Owner Name 3

Credit Card Number 10

Any idea what is causing this?
