Product Listing - With Custom Text Input For Customers

I am trying to find a way to add the ability to have a 50 character line or shorter, where people can type in a message or name for engraving on some of the items I sell. Is there a mod that adds this ability to a product listing, so people can select the item and type in what they want?

i've used the image upload contribution before. http://addons.oscommerce.com/info/1540

 

it lets you to set up attributes that will allow your customer to upload an image, enter some text, etc.

 

it's coded for ms2, so if you're using rc2 it will take a little effort to get the code to work correctly. the mod also does not make changes to the invoice, so if you want the text/image to appear on the invoice you'll need to do a little more work. but it's a good start.
